AbstractThere is a process within the special education program that directly links a child's educational program with their plans for the future. It is called a Transition Plan and is part of the Individualized Education Plan (IEP). Transition services are a coordinated set of activities for a child with a disability designed to be used within a results-oriented process. The IEP team will meet to complete the transition plan section of the IEP. The transition plan supports the child's transition into adult life, based on the child's individual needs, strengths, interests and preferences. The focus is on improving the academic and functional achievement of the child with a disability to facilitate the child's movement from school to post-school activities, including postsecondary education, vocational education, integrated employment (including supported employment) or a setting typically found in the community and employs individuals with disabilities and non-disabled individuals at all levels and individuals with disabilities, continuing and adult education, adult services, independent living, or community participation. This document provides descriptions and guidance on: (1) secondary transition; (2) results-oriented processes; (3) who should be involved; (4) transition age 14 years; (5) IEP Section 5--postsecondary transition; (6) Section 5--postsecondary transition IEP form; (7) course of study; (8) transition assessment; (9) IEP development and transition; (10) postsecondary goals in the IEP; (11) Section 5--postsecondary transition services sample of the IEP form; (12) goals; (13) transition services/activity (action plan); (14) self-advocacy; (15) transfer of rights; (16) alternatives to guardianship; (17) summary of performance; (18) "backwards" planning; and (19) resources.
