Abstract | The Youth Transitions Partnership (YTP) in Alameda County, California blends service coordination, intensive case management, and Dialectical Behavior Therapy (DBT) to help transition-age youth in foster care engage with available support systems and subsequently improve their outcomes. As part of a formative evaluation of YTP, Chapin Hall researchers found that participating youth experienced significant changes in the areas of employment, financial literacy, and permanent connections. They also experienced significant gains in their acquisition and use of DBT coping skills. This brief describes the YTP model, the Continuous Quality Improvement (CQI) process that was used to support its implementation, and findings from our formative evaluation.
