AbstractThis portfolio project reports on an effort to help Utah school districts prepare students for a smooth and successful transition out of high school and into post-secondary education or the workplace. Through the project, three important school processes were identified: (1) career guidance; (2) long-term planning and goal-setting; and (3) maintenance of related student work in a portfolio system. Based on information collected through the project, a model was developed and disseminated as an information resource to Utah educators. Components of this model include training students to make decisions and set goals for academic success from elementary school to high school through Student Educational Planning and Student Educational and Occupational Planning processes, continuous advisement, coursework, and recording progress throughout the school years in a working portfolio as well as culminating achievements in an exit portfolio. The model also includes suggestions for designing the system at a district or cluster level. Includes reports on each school visited. Through these site visits the operation of nearly every component included in the model was observed. For each component, recommended procedures for implementation are included. (MKA)
