Abstract | This final report describes activities and achievements of the EASE (Equal Access for Students to Education and Experience) Project, a 3-year model demonstration project on transition of students with disabilities at seven community colleges in the Arrowhead region of Minnesota. The program served 550-600 students and focused on providing support for the transition to postsecondary education, for increasing success in college, and for transition to employment or a four-year college. Among many activities and services of the project listed are collaboration with secondary schools, referral and collaboration with community agencies, provision of parent information and workshops, development of individual education plans with each student, provision of accommodation provisions on an individual basis, small group and individual counseling, technology access and training, and partnerships with employers. The report also provides a list of publications produced by the program, a detailed timeline of project activities, a breakdown of project achievements by objectives, the dissemination plan, student surveys, letters of support, and plans for continuation of many project activities. Various project deliverables are also provided, including: (1) "The EASE Project Snapshot: Procedures and Replication Manual"; (2) "The Answer Book: Job Search Strategies for Students with Disabilities"; (3) "Employers' Handbook for Workplace Accommodations for Employees with Disabilities"; (4) the "Opening Doors to Learning: Tutor Resource Manual"; (5) "College Success: A Handbook for Students with Disabilities, Mesabi Community College"; and (6) "Students with Disabilities: A Resource Guide for Mesabi Community College."
