Abstract | To assist work adjustors, evaluators, and counselors in learning how to write individualized work adjustment plans, this publication describes one method of developing individual client plans--the Materials Development Center Individualized Work Adjustment Plan. The detailed description of this plan explains the two basic parts of its format: (1) "page 1," on which are listed and prioritized problem behaviors or skill deficits, and (2) the individual goal sheets which set forth each behavior to be worked with in the adjustment program. Suggestions for using the form, sample forms, and example plans are provided. Appended materials include a work adjustment program evaluation checklist and a simulated discussion between the director of client services and a rehabilitation consultant; the discussion describes the adjustment plan's purpose, identifies problems in implementing work adjustment services, demonstrates steps in developing individualized plans, and emphasizes the need for a written plan for each client.
