Abstract | This study, one in a series of qualitative case studies on state and national practices in deinstitutionalization and community integration of people with disabilities, was conducted in 1991-92 and involved semistructured on-site interviews with administrators in the 12 New Hampshire regions. The focus is on two key milestones in the development of the New Hampshire community service system: first, the concept of area agencies and, second, the use of the home and community-based Medicaid waiver. This case study has three primary sections. First, it briefly presents an overview and analysis of the regional structures, especially the tension that characterizes the relationship between the regions and the state Division of Mental Health and Developmental Services. Next, the study describes application of the home and community-based Medicaid waiver, and contrasts it with intermediate care facilities (ICFs) for people with mental retardation. The third section presents a framework for thinking about regulations. Conclusions include the importance of local control, the need for intermediary structures to provide necessary flexibility, and the flexibility and cost-effectiveness of the Medicaid waiver in moving individuals with disabilities from institutional to community life. An appendix provides more detail about three regions in New Hampshire: Region V, Region VI, and Stafford County. (DB) |
